If you have been ill for more than 7 days, you may be asked for a doctor’s certificate; these certificates are called ‘Fitness To Work’ Certificates or a ‘Fit note’ for short. For more information, please see NHS: When do I need a fit note?
If you have not yet spoken to a doctor about this illness/condition, you will need to book a telephone consultation with a GP or ANP. Please book an appointment with a GP/Carley Moore. Note: if there are no appointments available, please try again from 7am on the next working day.
If you have already been seen by your Doctor about this illness, you can make a Fit Note request. If a doctor wishes to speak to you again before issuing the Fit note, they will message you by email or text. Note: please only request a Fit note extension when your current fit note has expired. Fit notes can be backdated but a doctor cannot post-date a Fit note (the doctor cannot create a Fit Note to start in the future).
